Observation

110-300-0145(1) Licensee had a cigarette butt in the licensed play space, an opened accessible large grey garbage can with the lid open with soiled diapers and garbage accessible to the children. There was large screen door that was laying on the front side of the house. In the outdoor space there was two quad vehicles accessible to the children. 110-300-0145(11) Licensee did not have a working self-closing or self-latching gate on site. 110-300-0145(8) Licensee had several areas of the outdoor fenced licensed space that had gaps of three- and one-half inches or more, one gap was on the right-hand side of the garage, second gap is by the gate in the front of the licensed gate, and the third and fourth gap is by the gate in the back of the home by the gate in two places.
